POST TITLE On Call Care Manager
RESPONSIBLE TO Managing director and Operations Manager
MAIN PURPOSES
To provide high quality, professional service to Service users, staff, colleagues, health service professionals and others. To act as the first point of contact for service users contacting the office and to portray the company in the highest possible professional basis. To ensure that enquiries from service users are efficiently and courteously handled to the mutual satisfaction of all parties.
To carry out care tasks in accordance with social care codes of practice and company policies to ensure the delivery of a sensitive and efficient service that enhances the quality of a service user's life.
MAIN DUTIES AND RESPONSIBILITIES
Risk assess all new service users within 48 hours of care plan commissioner
Dealing with social services and other professional bodies
Dealing with service users both over the phone and in person and attending reviews
Check, amend and review all service users ensuring IQ Care planner is always kept up to date.
Covering on call every other weekend and throughout the weeknights
Receive and accurately record requests for cancelled calls, changes to planned times, respite care, and hospital admissions. In accordance with the company’s protocols
Assist to ensure IQ Care planner is always up to date.
Answering incoming telephone calls, ensuring calls are documented and redirected accordingly.
Ensure telephone calls are dealt with quickly and directed to the correct person. Care staff should not be encouraged to tie up the phone lines.
Set-up of service users’ files
Advised service users of relevant charges for private services,
Produce diary notes as requested keeping track to prevent duplicates – double check service users’ names are spelt correctly
Ensure correspondence, reports, etc. are filed in correct service users’ records.
Undertake administration tasks as allocated by senior staff members
Medication delivered to the office is entered the log and is delivered in a timely manner and carers are informed to collect from office and if not able deliver.
When needed cover calls, covering sickness, holidays, training at short notice
Always work safely in accordance with Legislative requirements and Company’s Policy and Procedures.
When dealing with difficult situations ensure advice / support is gained from the MD and Care director
